Adolescent Program Eligibility

The following will be used to determine eligibility:
1) Application, 2) Criteria for Admission, & 3) Supporting Documentation
Exclusionary Criteria for the adolescent group home includes:

  1. Individuals diagnosed with an intellectual disability or developmental disability

  2. Individuals who are actively suicidal, psychotic, or homicidal

  3. Individuals who require inpatient, medically monitored detoxification services

  4. Individuals younger than age 14

  5. Individuals who require medical care beyond the capabilities of the program which could include daily nursing services, specialized medical/nursing procedures, specialized medical devices, specialized feeding services, etc.

  6. Individuals required to register on the Sex Offender registry or have not completed Sex Offender Treatment Programs

  7. Individuals convicted of violent crimes

Please provide the following when applicable:


  1. Custody order

  2. Current behavior treatment plan

  3. Educational records and most recent school transcripts / Current IEP

  4. Psychological (and/or other applicable testing)

  5. Medical records, including current insurance information

  6. Current physical (not older than 90 days or within the last 12 months if transferring from another state-licensed facility)

  7. Current dental exam, completed within the last 12 months

  8. Current Immunization record

  9. Progress notes and discharge summaries from past placements

  10. Legal history and involvement
